病原体名称: 多重耐药铜绿假单胞菌 (MDR-PA)
生物学特征: 革兰氏阴性杆菌,具有运动性(单极鞭毛),大小约为 0.5-0.8 μm x 1.5-3.0 μm。
耐药定义: 对至少三类抗生素(如氨基糖苷类、碳青霉烯类、头孢菌素类、氟喹诺酮类等)中的至少一种药物不敏感。
主要耐药机制:
外排泵: 如 MexAB-OprM 系统的高表达。
外膜通透性降低: 如 OprD 蛋白缺失或表达下调,导致对碳青霉烯类耐药。

Pathogen Name: Multidrug-Resistant Pseudomonas aeruginosa (MDR-PA)
Biological Characteristics: Gram-negative, motile rod (single polar flagellum), measuring 0.5–0.8 μm by 1.5–3.0 μm.
MDR Definition: Non-susceptibility to at least one agent in three or more antimicrobial categories (e.g., aminoglycosides, carbapenems, cephalosporins, fluoroquinolones).
Primary Resistance Mechanisms:
Efflux Pumps: Overexpression of systems such as MexAB-OprM.
Reduced Permeability: Loss or down-regulation of OprD porins, specifically impacting carbapenem susceptibility.
Enzymatic Inactivation: Production of β-lactamases and carbapenemases (e.g., NDM-1, IMP-type, VIM-2).
Epidemiology in China: Prevalence has increased over time, notably in ICUs (est. 29.0%). Predominant high-risk clones (HiRiCs) identified in mainland China include ST1971, ST244, and ST357.
Diagnostic Standards: Susceptibility testing is typically performed using broth micro-dilution methods according to CLSI Guidelines.
